help for filling up Visa application

My question -
I am having a job offer with a Certificate of Sponsorship in UK and I wish to apply for a Visa for myself and my dependent wife and son. I will be working in the NHS Hospital in UK

On login onto the website, in the first page I need to fill the Visa type and subtypes. I am confused and need your help as what should be the appropriate Visa type, subtype for me and for my dependents.

If you visit the website https://visa4uk.fco.gov.uk/, then on the first page when you apply for visa, there is a Section "Select Visa Type"

1. REASON FOR VISIT - If i select WORK for myself, i get the following options. What do I select for my wife and son.

2. VISA TYPE - the options in the drop down box are as follows. What do I select for myself and what do I select for my wife and son.

Would request you to please look into the options carefully and tell me which one should i select for myself and which one should i select for my dependent wife and son.

you are the primary applicant so first you will fill the form for yourself and later they will ask details for your dependents.
You should probably be on Tier 2 General
Whether on not your post is on the Shortage occupation list you should check your COS for that, it will be mentioned. Also how long your COS is for, if its 3 or more years you can apply for more than three years or if its less than that you can apply for less than 3 years.
