First passport application please advise

Hello senior members I am going to submit passport app tomorrow. Filling up form now and got stuck with few things.

1 section 3 I understand I need to send native country passport do I need to send expired passport as well ?

2 section 10
Person going to fill up section 10 same person need to sign back of my photograph ? He need to write his full name and sign or just signature please advise

1. Current valid passport.
2. It says in the form the exact wording of what your countersignatory should write on the back of your photo. It also says it should be signed as far as I can remember.
