Administrative Review question

Hi,

Recently my T2 General visa was rejected. Reason stated was that I didn't submit original to prove my English proficiency. But I have kept duplicate copy on the advice of collection officer. I am going to appeal stating the same, below is my understanding about the AR process, please advice in this. I am not sure about the success but I feel its worth a try.

1. I should state the reason why I need a review in the petition.
2. Should I need to send any document along with it (originals, application form, photo copy of any document, etc)?

Any advice in this regard will help me a lot.
