Form SU07 who shoud certify?

For family visitor visa (my mother to come for 1 month) needed Form SU07/12 . There on the second page "For official use only" who shoud certify it?

I'm sure i've seen somewhere in requested documents for family visitor and It said in description that it must be provided. However, who should certify it?

It doesn't need certifying if you're giving them the original.

I'm really sorry Amber but I confused now. Should I provide SU07 form or not? if provide should I give first page only?

Any one please share your experience for recent family visitor visa documents provided and outcomes.

You should provide both pages but the 2nd one if for official use only.
