Fiancee and/or Spouse Visa question

Hi there,
I am a British citizen living in the UK. My fiancee is in the USA. We met while I worked there on an intercompany visa but I had to come back. Unforntuanetly, I cannot travel to the US as I was declined a visa. My questions are:

1) Can my fiancee apply for a UK fiancee visa in the US to come to the UK without me being present at the interview in the US?
2) If not, can my fiancee apply at another embassy in a different country from her residence where I can attend
3) If we were to marry in another country, can my fiancee (then wife) apply for a spouse visa in the US to come to the UK without me being present at the interview in the US?

Any help and/or information appreciated.
