New categories in fees for Ties 2 general

Dear All
In the new forms of Tier 2 general, there are new categories for fees payment. I am bit confused which one will be correct category for me. I am in Tier 2 general for last 4 years which will be completed in oct 2014, so I have one more year till oct 2015 to apply for ILR. The first category for payment which is mentioned is :
:Tier 2 General, ICT Long Term Staff, Sport & MOR - up to 3 years leave to remain
The other one:
:Tier 2 General, ICT Long Term Staff, Sport & MOR - up to 3 years leave to
remain - CESC
I don't think that I have to go for over 3 years category as I am due for ILR next year and my job is not in shortage occupation list anymore, I am working as staff grade surgeon. My job was in shortage occupation in 2010 when I first came.
So I guess I will have to pay in first category which is Tier 2 general upto 3 years leave to remain. Correct me please if it is wrong.
What is CESC?
