fees and application form needed to sponsor a child

Hi all

I have Discretionary leave to remain. This visa permits to sponsor dependents if there is compelling compassionate circumstances. I want to sponsor my child (under 18) from Pakistan.
My questions is what application form do I need to complete and how much is the visa fees. I can't find the relevant application form as all applications refer to settlement permanent visa while I have discretionary leave which is limited leave.

What exactly is the leave you have? DLR for 3 years or private life leave for 2.5 years?

I was granted 3 years DLR in 2009. Then another 3 years DLR extension in 2012. I will get ILR in 2015. By the time I get ILR my child will be over 18.

Where is the other parent? Who has responsibility for the child?

The child lives with grandparents. I have the responsibility, my wife died 5 years ago.
