Help Required -Applying for Naturalisation

Hi

I am in the process of applying for naturilasation and am unsure if i need to put my surname as it is in my Indian passport but my surname has now changed as i got married 6 years ago.

I use my new surname for all my banking etc at present.

Not sure if i need to send my wedding certificate with my application.

Any assistance will be appreciated.
