Urgent Advice: Tier-2 General Change of Employment.

Dear All,

Presently i am working in NHS and got another job and so will be changing employer to another NHS Hospital.
I am on Tier 2 General and will change to Tier 2 general as recently got CoS from the new hospital which is valid for 1 year ONLY.
When i was trying to book for Premium, in the drop down menu there are so many categories, which i dont know to select.
Can someone please advise.
