Assuming you are a non-EU or other visa-disadvantaged national...
You need an employer willing to sponsor you, you need to speak very good German, and a lot of patience. My partner did this is 2006, she is no-EU, but she speak near-fluent German so that bit wasn't hard, just the length of time it took, nearly had to cancel it (it was university placement for German language study). The work permit was applied for by the Germany company willing to offer the placements.
You can't apply for it yourself.
