Tier 2 - application for extension for 5 years (Urgent)

Hi

I am a tier 2 visa holder and I am due for extension after initial 3 years of continuous stay Nov 2011- Nov 2014. My employer is sponsoring me for 5 more years and wants me to apply this week.
I have questions for you please:

(1) Can I apply for an extension of 5 years or do I need to apply for 3 years only?
(2) If I do apply for 5 years extension, will it get rejected if they can only give me 3 years, or will they give me 3 year visa in any case irrespective of whether I apply for 3 or 5 years please?
I have to spend £1202 for a 5 year extension vs. only £601 for a 3-year extension Is it worth to go for 5 years? Please help! Thanks a lot in advance!

have they already assigned an unrestricted CoS to you and if so what is the work end date in it? Suggest you get a printout
