Tier 2 (General) Documents Checklist

Hi.

There are many posts about dependent documents list but I want required documents list of an individual who is applying on tier 2 visa.
The documents list I have found on UKBA website are:

one passport size photo
proof of English language proficiency
passport
bank statements ( not required if a rated sponsor is assuring for one month accommodation)
application form
TB certificate

someone please confirm me if any other documents are required ?
do we have to provide COS Summary Sheet ? for proof of english language do we have to submit original report ?

Do I need just the CoS number to apply for a Tier 2 visa, or an offer letter is also necessary to be submitted along with the visa application? I do have an offer but the HR has still not prepared the contract- may take some more time. Can I apply for a visa in the meantime just with CoS number?
