Tier 2 (General) form- question C3 ?

Hi, I am applying to change employer under Tier 2 (General). What do I write for question C3 in the form:

C3. Do you have any passport or travel document which contains a previous grant of leave or which you used when you last travelled to the UK ?

I entered UK under a Student visa stamped on my 1st passport. The same passport was later used for my Tier 1 PSW visa. That passport and my BRP for PSW were later stolen when I was on holiday abroad. I got a new passport there which was a short-term validity passport to return to UK. Once in UK, I used the 2nd passport to apply for a replacement BRP so my PSW visa on my BRP was basically now linked to the 2nd passport.
I later applied for and got a new passport (my 3rd one) which I later used to switch from PSW to Tier 2 (General) in 2013 for which I got a new BRP as well. My current passport is my 3rd one. So, my current Tier 2 visa is linked to my current (3rd) passport while my old PSW visa was later linked to my 2nd passport.

I have already entered details of my current (3rd) passport in question C1 and of my 2nd passport (the short-term validity one) in question C2 and am enclosing both with my application form. So, for question C3, should I choose 'No' as I am enclosing both passports used for my BRP's or choose 'Yes' and enter details of my 2nd passport (which I think is the correct answer) or choose 'Yes' and enter details of my old, stolen passport (as that was the 1st passport I used to enter UK on a Student Visa] ?

In ques. C3, should I fill out my prev. passport's details again as that passport had my old, expired Post Study work (Tier 1 PSW) leave ???

I will be enclosing my current passport and BRP that has my current Tier 2 (General) leave (from Jan. 2014- Jan. 2017) on it. I have entered it's details for question C1. I will also submit my old passport and have provided it's details for question C2. That old passport was used for my old Tier 1 Post Study Work (PSW) visa which expired in Jan. 2014.

For question C3 on the form:

Should I fill out my old passport's details again here as that passport had my previous grant of leave (though the PSW leave was "on" the old BRP) ? The last passport I used to travel abroad for a holiday and return to UK was in April 2014 and it was my current passport and BRP (under Tier 2 (General) leave) ?
