6 Years Discretionary Leave

Hi! I am new to this forum and can someone help me with my question. I am due to apply 6 years discretionary leave last May 12, 2014 but i was out of the country. I'm starting to fill up Set (O) before my visa expires in Nov 8, 2014 is this the right form for me?, will this affect my application for ILR cause i didn't apply in May? Thank You
