ILR Application SET(O) Tier 2 Migrant Queries

Hi,
I am applying for ILR, while filling up SET(O) form as Tier 2 Migrant had some doubt and looking for help urgently please.
Form Section 5 (Page-18):
How can I answer these following two questions?
1. Current rate of pay Per Hour (as stated in the codes of practice –annual salary)?
2. Number of hours this annual salary is based on?
As I am a permanent employee and my employer provided me SOC Code, Annual Salary and 'Numbers of Weekly Hours' but they did not give the per hour rate of pay as well as total yearly hours. According to my employer reply as a permanent employee so the hourly rate is not applicable.
Is it advisable to calculate and filled up the above two?

From Section 6 (Page-21):
1. I have listed all leaves (holiday outside UK) for 5 years from to date, there are few days (11 days) which was Loss Of Pay not paid leaves, is that going to be a issue for me, if yes then what should I do?
