pio application form question

Hi, I'm trying to fill out the PIO application form to take to the HCI in London, and I'm a bit stuck on the best way fill out a couple of the questions. I was born in the UK, and my (late) father was born in India but became a British citizen. In question 2 (a), your father's nationality is requested: should I put Indian, his nationality at birth, or British, his nationality when he died? Secondly, in question 13, details of parents passport is requested. My father had to surrender his Indian passport when he became a British citizen, and I don't have any of the necessary details about it. Is it best to just leave the box blank, or should I write in the details of the evidence I do have, his school certificates etc.? I don't want to queue for hours at the HCI, only to have the application rejected because I've written the wrong thing in one box! Many Thanks in advance
