UK tier 2ICT cooling off query

Hi,
I was in UK from Dec'10 (visa stamped in nov'10) to 9Nov'13. Initial visa was valid for 2 years and it got extended for another year from UK itself until 23 Nov'13. Now my employer wants me to visit UK again on tier2 ICT (short term) from India
Please answer below questions:
Whether there is 12 months cooling off period applies to me?
If so Can my employer start processing a fresh COS now although the start date they put would be after 9Nov'14.
When can i visit embassy for entry clearance at the earliest and what document i need to carry to proof i came back on 9Nov'13 (Is passport stamp suffice)?

yes, it does

the CoS can be assigned now as it is valid for 3 months but you cannot apply before 24 Nov 2014 or, if you left the UK before the expiry of your visa, 12 months after your departure date. The work start date in the CoS should not be before the date you are allowed to return to the UK

and what document i need to carry to proof i came back on 9Nov'13 (Is passport stamp suffice)?
if you apply on or after 24 Nov 2014 you don't need any documents but if you want to apply between 9 and 24 Nov it would be helpful if you could produce airline ticket stub and any other evidence you may have available

My COS start date is 8Dec'14 but i'm getting an appointment at VFS for entry clearance on 9Dec'14(not before) Can you please suggest should i go ahead with appointment or do i need to get my COS start date changed again? please help urgently
