Tier2 visa - Change dependant status

Hi experts,

I have extended my Tier2 Visa in September for 3 years and did the same for my wife (ex) and as things did not go well between us we decided to separate and therefore I want to inform the Homeoffice about the change of her circumstances and probably they have to revoke her visa. I could not find anything on HomeOffice's website regarding to my situation.

So I am wondering whether there is a formal form somewhere I have to fill and send to homeoffice? If not is there any letter template so I can use to write to them?

What if you change your minds and want to be together again? Is there a need to rush?

In this page said both sides need to inform the HO. I can write to them myself but what if she decides not to write HO?
Is she going to have any legal issues?
