Indian PIO card applicaiton

Hi can anyone help me. My mother who was born in India and married my father who was a British citizen and went to live in Uganda,and then came to live in the uk in 1972 during the exodus. My mum who does not have an Indian passport as it was left behind in Uganda wants to apply for OCI OR PIO but the Indian embassy returned her documents saying she needs proof that she was born in India. She has a Indian birth certificate school leaving certificate but now they are saying she needs a nativity certificate. Can anyone help with this, where I can get one or is there any other method to solve this long process as she visits India 3or 4 times a year.
Any help will be most appreciated.

Hi this is what she sent a year ago
Birth certificate stating her indian birth
School leaving cert
British passport
Letter to say she has no indian passport as it was left behind in uganda.
Do I need to get a notary affidavit ? If so can this be done from the uk and do you know where I can get this from
