Tier 2 G premium service questions.. please help

hi guys,

this might sound like an obvious question, but since there is so much riding on the success of the application I want to make sure the details are right.

I am planning to apply for a Tier 2 G change of employment visa through an in-person premium service. I wanted to know if I have to fill in a paper application to submit it during the appointment or submit an online application?

If its a paper application, is this the right form to use:

Is the same process applicable for the dependant application as well?

Should two seperate appointments be booked for the premium service i.e. one each for main and dependant applicant?

Yesterday I have asked the IT support team about the correct paper form for a premium service, you are using the correct paper form. I think you can bring dependents with you in the booked premium service and you should submit all documents and forms for you and all your dependents. BUT when booking the appointment you should select how many dependents you will bring.
