Tier 2 General Visa Candidates Salary

I was wondering some one could help me in the following Subject.

1) I am bit Confused regarding the salaries Tier 2 Employees get Paid.The COS Issued to a IT Professional will show 33,600, but most of the Tier 2 Employees get paid more than thast. RECENTLY ONE OF MY FRIENDS Company got Suspended in that suspension they have raised a question regarding the highly paid salaries.
If we update the salary (In General Paying around 50,00/yr) in the SMS is it ok or else we need to do RLMT again to pay more Salary to an Employee.

If there is a change in SOC code even within the same employer RLMT must be done and a new application submitted.
Please provide more details on exactly what happened? Did the employer raise the salary but did not perform RLMT and do a new application?
