In-country switch from student visa to WP - ILR start date?

Experts, please advise.

I was on a student visa from September 1999. I got a job and an in-country switch to work permit in February 2003. I complete five years for ILR in February 2008.

What should be mentioned as the start date in the ILR application - September 1999 or February 2003?

If it is February 2003, what should be the start date - the date of work permit application being granted or the date I started job or five years before until the visa validity (13 February 2008)?
