Restoring Health Insurance in an EU country

Hello Everyone,

I couldn't think of a better section to post this topic, but if there is one please let me know so I ask this one to be deleted and post a new one in the correct place.

A couple of years ago, when I moved to the UK and started paying my National Insurance Contributions here, I suspended my Health Insurance back in my home country - Bulgaria.

I am looking to re-instate my health insurance back in my home country, so that I can get some specialised treatments when I am back home on holidays, which I cannot receive otherwise.

I do have an European Health Insurance Card, however this is not the equivalent of having my Health Insurance status restored in my home country.

In order to do that I am required to present E104 forms for the past 3 years.

I tried looking up on the internet how to obtain one, but I came across articles saying that it is no longer available.

I even found the E104 page to be archived and no longer relevant.

Can you please help me by telling me which new form is the new equivalent of E104 and what is the procedure to obtain it?
