EEA PR application / section 9 self employment

Please I hope you can help me, its kind of a silly question,

I am a family member of an EEA, who is self employed, in section 9, (3) it says

9.6 For any period in which you were, self employed, please give details below:

Name and address of business / type of business / date started trading / date stopped trading

Should I put the name and address of his business, or the name and address of the business he is working for as a self employed?
