Clarification of Previous Earnings - through Ltd com

I have been working in the Uk as a contractor, being posted through agencies.
I am looking to apply for the HSMP.
I have printed the invoices from the agencies, and had them stamped and signed, along with an earnings statement.
Will this be sufficient, or because I have been paid through a limited company do I need to include other documentation.
Also, what section am I to fill in - salaried employment, or contractor or self employed. I have worked at two different companies as a contractor for both 6 month contracts as an accountant.
I have reviewed the forum and taken valuable notes, but there doesnt seem to be anything covering this topic,

If you are applying for a HSMP now then what is your current status............apologies if this might sound adrift from your original query but I think it matter to your case as say if you are on work permit then you should be a employee and should not have invoices instead you should have P60's and P45's........

I am sure you have covered all this but I though I should point out........

I am working on a Working Holiday Visa, but my work portion is now running out. Should I ask the agencies for the P forms?
Or who would it be my limited company provider?
