NHS Surcharge

Hi,

I have booked a Tier 2 (General) premium service on 1st April 2015 for 11 April 2015. I am now confused whether I have have to pay the NHS Surcharge or not. There was no option for me when I have booked the appointment online on 1st April 2015.

Please help..
