Switch fiancee visa into spouse visa Urgent please help

1/I saw on FLR(M) guidance that they said to apply for spouse visa we have to apply for 6 documents which got the both of wife and husband names
I came to UK by fiancee visa and i didn't add my name on gas bill or utilities bill yet .Because we lived in a council house with my mother in law name.
This is the first time i switch into spouse visa,do i need to apply that documents???it's hard for me to add my name now because my visa will expires on 25th May 2015.
2/Can i apply to switch into spouse visa after getting marriage certificate on 15th May 2015?I just saw that apply before visa expires,they don't mention maximium how many day i have to apply before expire???

You don't need to submit proof of 2 years co-habitation when switching from fiancee to spouse. You can apply any time after the wedding before the expiry date of your current visa.
You also need to pay the new NHS surcharge of £500 in addition to the FLR(M) fee before submitting your application.

I got a foreign divorce that officer told me i can sign or can't sign marriage certificate on 9th May...They need to get confirmation from General Registry Office.It can takes 2-3 months too
Now if i apply to extend my fiancee visa,do i need to pay for Healthcare Surcharge???
I contacted Homeoffice and they told me i can extend by FLR(M) form in that case,but i don't know how long they grant my visa (6 months to wait till the day sign marriage certificate)or 2,5 years.How much i have to pay for that???
