Please help me with Tier 2 Extension

HI this is Ven,
1) I need to renew my wifes visa too. DO I need to apply that seperately or can i make an single application

2) what are the required documents for visa extension.

3) My Visa expires on 08/08/2015. I came to Uk in 15/07/2008 my annual salary is 31,500 when is the best time to renew. suggest the dates so that i can get an ILR. later.

Please help me.....
