Individual ILR application for Tier-1 Dependent

Dear Team

I would appreciate for your expertise suggestions regarding my query for ILR application for Tier-1 Dependent

I am main applicant under Tier-1 General Migrant which was granted in July 2010 therefore I am eligible for ILR in June 2015 through 5 year route.

My wife arrived in UK in September 2010. since then she was with me and she want in and out to other countries but return with in 90 days

Now my question is if I go alone for ILR application in PEO(in person), then when i get the visa i would like to apply my wife ILR application via post. Therefore which application form should i use for her ILR application ?? and will she will be eligible for ILR ??

What documents should i show for her ILR application. She is a house wife.
