Help required for 5-yr route FLR

Hello, I would appreciate if someone could answer the following question regarding my further leave to remain application:

Q1: How long before I should apply for my further leave to remain as my first FLR (M) is expiring on 7 September 2015?
Q2: I want to apply through premium service, can I book my appointment online?
Q3: Do I have to pay healthcare surcharge for my another 2.5 years FRL (M), if yes, how much and how to pay this surcharge?
Q4: When my first application (FRL (M)) was approved for 2.5 years on 8 March 2013 I was given an expiry date 8 March 2015, which I thought was wrong and I asked UKBA to correct this to 7 September 2015 and they did at that time. Couple of months later I realised I made a huge mistake as the 2.5 years period starts on the day of application, am I correct? Is there any way I can ask home office to reduce 6 months on my second application for FRL (M) before I apply for ILR?

Why UKBA gave expiry date for only 2years instead of 2.5 on my first FRL (M)? And where do I pay NHS surcharge? Can I pay my normal FLR (M) application + additional for premium service and NHS surcharge at the same time? Many thanks.

I am the civil partner of a UK national and we are both living in the UK. Do I have to pay the NHS surcharge?

The UKVI made an error in giving only 2 years, which is why they corrected it when you complained.
Here's the link to paying the NHS surcharge. This is separate to the visa application. Once you've paid the surcharge you will be issued with an IHS reference number while you are online and will need to quote this in your FLR(M) application.
