Tier 2 dependant submitted wrong form

Hi,
I have completed wrong application form and paid the fees.
done tier 2 settlement instead of tier 2 dependant. I have cancelled the appointment now. and am not able to apply for refund..
When i click on apply refund.. its not doing anything. its highlighting on payment date and remains on the same page.

you can vary an application before a decision is made on it. Apply the correct way and write a covering letter explaining the circumstances and that you want to vary the previous application.
