I am filling form AN. My wife's name Irina Kuzminova. And maiden name Cernakova.
Section 1.5 Kuzminova
Section 1.6 Irina
Section 1.7. Do I need to fill ? :
Irina Cernakova or Cernakova only

If there is no middle name this looks appropriate based on names you have shown:
