This is a bit specific case of change of first name and i would appreciate if you could respond soon incase you may have come across something like this before.
I first successfully applied for Tier 2 General premium service for 2 years on 12th August and I was granted a BRP from 12th Aug 2013 to 29th Oct 2015.
In between, I have changed my first name in Aug 2014. As a requirement from home office, I applied for TOC - transfer of conditions application on 02 Aug 2014 which was returned as invalid due to fee payment failure. I applied for TOC again successfully and I was issued with a BRP with new name with Dates from 30/09/2014 to 29/10/2015.
Now, I am applying for 3 years extension of Tier 2 as a premium service. But I am confused about few questions that I need to answer in online and paper application forms.
1. Are you also known by another name? (in online form)
Should i put my previous name here even though I have ceased to exist with that name. In paper form it is clear where it asks "Have you ever been known by any other name?". But here question literature is not clear if this is for previous name or other current name?
2. What is your residence permit or visa start date? (Online form)
Now, my Visa was 1st applied on 12th Aug13 and the date on new BRP (after name change) is 30 Sep 14. What should I put as start date 12th Aug13 or 30 Sep14?
3. Have you ever been refused entry clearance/visa or leave to enter or remain in the UK? (Paper form)
Since my TOC application was 1st reurned due to non payment of fee. What should answer for above 'Yes' or 'No'.
