Tier 1 G Dep Extension Query

Hello every one,
I am going to apply for my wife's extension, I got the ILR through PbS tier 1 general, so I am going to apply for pbs dependent extension,
Could you please let me know maintenance funds required for last 3 months also are the funds should be in my account or my wife's account ? Or it doesn't matter ??? Also do I have to provide my employer letter with pay slips or my wife's pay slips?
