Switching from Tier 4 to Tier 2

I'm currently on a Tier 4 visa and have completed my degree. I have been working for an employer as part of my university placement scheme and they've now offered me a full-time job and Tier 2 sponsorship certificate for 5 years. I have got all the required documents stated in the guidance document (https://www.gov.uk/government/uploads/s ... ENERAL.pdf).

The documents being:
1. Sponsorship certificate
2. Degree certificate
3. Bank statement

Do I need any other document/letter along with my application? Secondly, my bank statement has salary transactions as I am working full-time with the same employer already because I've finished my degree and my current visa allows me to work full-time when it's not term-time; do I have to provide the evidence of my working hours in the form of a letter from employer?

Lastly, could you please also guide me on the Premium Service appointments, can I get an appointment within a week or so if I apply now?
