Form AN questions 2.4 and 2.5 / ILR

Hi! I really need some help to fill in these 2 questions. I am an EEA national exercising treaty rights (Italian), I came to UK in Jul 2003 and started working in January 2004 till Jul 2007 then I was unemployed till first week of December 2007, then worked till November 2009 when I was made redundant, then I was again unemployed till the 31st March 2010, however in this period I was 2 months in my home country (Dec 2009 to Feb 2010) then from 31st March 2010 until nowadays I never had any gap longer than 2 or 3 weeks. So my question is when did I got my ILR and when I was exercising the treaty rights? Finally how do I fill these 2 questions based on this information.
My proof of work from Jan 2004 till Jul 2007 is based only on a letter from the employer and bank statements as I don't have the P60s anymore.
