Urgent - EEA UK Family Permit Question for Wife and Child

Hi All

I urgently need some clarification. I am british and have to apply for UK EEA family permit for my wife and child. My question is, do I need to complete 2 separate applications for my wife and my child on this website:

https://www.visa4uk.fco.gov.uk/User/Home

If so, Secondly, shall I then need to book two separate appointments for biometric and submitting documents as well for my wife and child.

Are you applying under the Surinder Singh ruling?
(Casa, not CR001)

Yes - Applying under the SS route. Have been in Ireland for 3+ months now.
