NHS Surcharge-- How do I need to fill for Tier 1 general dep

Hi All,

I hope someone gets me out of this dilemma..

I have got a ILR.. and now I want to apply for my wife and child visa extension as a Tier 1 general dependent which will be for 3 Years. When paying NHS surcharge its calculating £1900 instead of £600.

Can anyone suggest please the options I need to chose.

It should be calculating £1200 (£600 x 2)
(Casa, not CR001)

Don't add your details just add your wife as main applicant and add dependent as your kid.
