Family visit form VAF1B

Does anyone know where one could obtain a PDF version of the form VAF1B for family visits?

With everything being mainly online now, its difficult to obtain PDF versions of the form. The idea is to complete the form in full prior to completing the final version online.

The Gov website only has the general one of the PDF (VAF1A), but not VAF1B and googling yeilded no results (only outdated forms).
