changing employer

hi,
my wife is currently on a general tier 2 visa working in NHS. She just got another role same position, but in a different trust. Can she extend her tier 2 visa and therefore change employer? or does she need a new tier 2? obviously her start date in new trust is now put back and being from australia she does not want to fly back home in order to apply again.
its really annoying that one can't phone the embassy for things like this. i hope anyone can give me their 2 cents/

done more research but still confused, how does one apply for 'change of employment' on the .gov website?
