HSMP Extension: Contracting with Limited Company

Hi ,
I am doing contracting with my limited company. Please let me know what are the documents required when i apply for hsmp extenion.
I am getting the salary (monthly) and i am getting eligible salary(40000/pa).

is the salary slips suffice for extension or do i have to given any other documents.

Please suggest what are the documents need for extension.
