OCI APPLICATION and required Documents

I just want to understand what documents are required for an OCI application in my situation.
My situation is:
I was born in india. came to uk in 1981 and obtained UK citizenship in 2004. further i sent my then valid passport back to indian high commision but never recived any form of confirmation. I do have an old cancelled passport that was issued to me in India and may have a copy of (i need to look to se if i can find it) the then valid indian passport that was sent back in 2004.

Can anyone help.

Applicants who have held an Indian passport, and obtained foreign nationality before May 2010, but are unable to produce the same for Surrender of Indian passport for any reason (lost/misplaced/submitted to UK Home Office long time back) are required to furnish a notarised affidavit in original from UK stating the facts about their Indian passports along with other necessary documents as mentioned below

Hello

I surrendered my Indian passport in 2004 and now need a deemed surrender passport. Does any one know the procedure and forms to use.

Phoned VFS and they don't seem to have a specific form for it in the UK but for USA their is a specific form. Seems a bit strange that they would not have a similar form for UK.

Why do you need a "form" to state facts? A notary will not notarise a "form", will he/she?
