What specifically are you looking to apply for?

Applications for permanent residence documentation are handled by form EEA(PR) now. It's a monster at 85 pages long, and replaces a bunch of forms including EEA3 and EEA4.

Applications for a registration certificate for EEA nationals are done on form EEA(QP) and for non-EEA family members it is EEA(FM). These were formerly forms EEA1 and EEA2 respectively.

Bear in mind there is no requirement to use any form, let alone the latest one, so you can still use the old forms if you can find them. There is an archive page of files on the gov.uk website where you can get them all.

Suggest check on Gov UK website - that's where they'll be.
