Discretionary Leave

Hi,
I was given discretionary leave for 3 years back in 2012 and then it was extended for another 3 years till the end of january 2018. I have 3 dependents and wife on the same leave. We had the discretionary leave on human rights after staying in uk for about 9 years.

I need some advise when i apply next year will i get ILR after completing my 6 years discretionary leave or will they put me for 10 years route. The law changed for few years back but does it still apply on us or we can carry the previous law where afrer 6 years of discretionary leave you get ILR.

Also my one daughter is 11 now and she was born in uk does she get nationality straight away when we apply again? Do i need to fill FLR (fp) application or FLR(o)?..

And last question is do we all need NHS medical for application?
