Birth certificate (India) query

What is the format of birth certificate and who issues the birth certificate in india ?

I was born in jammu and kashmir state.

Does a letter from the hospital where i was born is enough ?

Something like this..

"
To whom it may concern

As per our hospital records Mr XXX was born on dd/mm/yyyy

The local municipal corporation registers and issues birth certificates.
