Completed 6 years qualifying period (DLR)

Dear respected admin and all,

I hope all are happy.

I am new here and need your kind help.

I have been completed my 6 years qualifying periods of DLR on 01 Oct 2018.

Could you please let me know the procedure and new forms, I want to apply Premium Service (someone said this service is closed now on 29 Nov 18 but some limited slots are available till 9 Jan 19), if I apply new system which is the proper form (online) for quickest service. We have total 6 peoples (myself, wife and 4 children).

Could you please let me know the procedure of by post application. I am very grateful if anybody help me for by post application form (form name and which is the mail address of Home Office).
