Employee 2:- Employed (Part-time) : March 2018 -Feb 2019 {Total Hours Worked = 1471.51}

Employee 1:- Employed (Part-time): July 2018 -June 2018 {Total Hours Worked = 766}

I have raised my concern with my Accountant and he indeed confirmed that there were mistakes in what he gave me and he is working to correct them and get back to me.
Basically the online form only allows you to mention one wage rate. If your employee had multiple wage rates during your period of claim, its best to draw up a table and upload with your documents or mention in cover letter. will you be attaching a wage rate, hours etc = I believe all the are covered in the RTI and Payslips

Feel free to list all three employees as 3 separate jobs!

10.Payslips (Mar 18 to Feb19) for all employees
11.PAYE reference letter (not necessary but if you have it then do so)

8. Evidence of immigration status in the UK
9. Signed Declaration to confirm consent for the Home Office to request verification checks
10. Current Biometric Residence Permit
11. Documentation to prove the employee was a settled worker for example, the biometric page of a British passport showing the employee's photograph and personal details
12. HM Revenue & Customs documentation to demonstrate your registration
13. Wage or pay slips to cover total period of employment created for each worker
