ILR application for child under age 2 years

Hi,

I am applying for ILR using SET(F) application online for my son who is 2 year old now. Me and my wife, both are having ILR. Does any one have template for getting a letter from hospital/GP to state or show "Evidence that my son live with me (sponsor)?

If child was born in the UK, then child is also directly entitled to register.
