parents consent form

Hi there everyone,
As ever, thank you for all your help especially the wonderful guys who take time to respond regularly to people's questions, you won't know how many of us you've helped!
I'm submitting naturalisation applications for myself and my son on Monday but can't find anything for a parents consent form? Can I write one myself with all the info required. Read somewhere that some receive it with the referees form but I didn't. Anyone able to advise on the way forward?
