Visa health surcharge

Good afternoon
Sir /madam
The mother of my children had applied for entry clearance to the UK under a tier 2 general and pbs dependent visa she was granted entry clearance and our dependents were refused under immigration rule 319h they stated that the mother does not have sole custody and that they did not see any application for me and that i can take care of them until I apply she is currently residing in the UK. I made an application for them and myself under my COS I was granted entry clearance, however during there application I noticed the health surcharge for both of them was 0.0 dollars we were still issued ihs reference numbers , I also noticed that the previous surcharge payment was refunded to my card I made several calls to the home office information center they told me to go do the biometric as per Normal if they need the surcharge payment they will contact me as part of the application process, however they never contacted me by call or email and I noticed my children passports have been dispatched and is returning via FedEx my question is could this mean my dependents Visa or entry clearance have been refused ?
